1. Direct Pay
Eclosion Health, PLLC ("Eclosion") is a direct-pay medical practice.
Patients pay Eclosion directly for its services.
Eclosion does not participate in commercial health insurance networks and does not submit commercial insurance claims on a patient's behalf.
Patients enrolled in Medicare or another government health program may be subject to separate federal or program requirements. Eclosion asks about applicable government-program enrollment before scheduling and will explain before care is scheduled if those rules affect the requested service.
2. Fees
Eclosion's current treatment fees are published on the website and are confirmed before a service is scheduled.
For services identified as episode-of-care pricing, the quoted fee includes the components specifically described for that service, which may include physician review, the house call, evaluation, treatment, supplies, and routine treatment-related follow-up.
The fee applicable when the service is booked will apply to that scheduled service unless the patient and Eclosion agree otherwise in writing.
If additional or different care is recommended after review or examination, the patient will be told the applicable charge before that additional service is provided.
Where federal Good Faith Estimate requirements apply, expected charges will also be reflected in the patient's Good Faith Estimate.
3. Payment
Payment timing and accepted payment methods are disclosed during booking.
Unless Eclosion agrees otherwise in writing, applicable treatment fees are due before or at the time the service is provided, as stated when the reservation is confirmed.
Eclosion may decline to schedule additional nonurgent services while an undisputed balance remains unpaid, where doing so is clinically appropriate.
4. Cancellation, Rescheduling, and Missed Visits
A mobile reservation sets aside physician time and preparation.
Unless different terms are disclosed before booking:
A patient may cancel or reschedule without a cancellation charge with at least 48 hours' notice.
For cancellation with less than 48 hours' notice, a missed visit, or a visit that cannot proceed because access to the agreed location cannot be obtained or because of a condition at the location within the patient's control, the cancellation charge disclosed during booking may apply.
If Eclosion postpones, relocates, stops, or cancels a visit for a clinical, safety, operational, or other reason not attributable to the patient, no cancellation charge applies for that cancellation, and any amount prepaid for a service that is not provided will be refunded or credited at the patient's election.
5. Refunds
An amount paid for a service that is not provided will be refunded, less any valid cancellation charge that applies under this policy.
If a procedure is begun but must be stopped for safety before completion, the patient will be charged only as disclosed before the procedure, and any prepaid amount above the applicable charge will be refunded.
Refunds are ordinarily returned to the original payment method.
6. Insurance, HSA, and FSA
Patients may request an itemized receipt for services provided.
Eclosion does not guarantee that an insurer, health plan, health savings account, or flexible spending account will reimburse or recognize a particular expense, and payment to Eclosion is not contingent on reimbursement.
7. Outside Providers
Pharmacies, laboratories, imaging centers, hospitals, emergency medical services, therapists, and other outside providers are independent of Eclosion and may charge separately for their services.
Their charges are not included in Eclosion's fees or Eclosion's Good Faith Estimate unless expressly stated.
8. Good Faith Estimates
Patients who are uninsured or self-pay as defined by federal law generally have the right to receive a Good Faith Estimate of expected charges when the federal requirements apply.
Eclosion provides Good Faith Estimates upon scheduling and upon request within the timeframes required by federal law.
Nothing in this Financial Policy limits a patient's rights concerning a Good Faith Estimate or the federal patient-provider dispute-resolution process.

9. Questions
Questions about a fee, charge, or this policy may be raised with Eclosion at any time and will not affect the patient's care.
