We review before we arrive.

Your history, prior imaging, and goals are reviewed so the visit can be planned appropriately. Telehealth evaluation is used when helpful.

We come to you.

A physician performs the in-person evaluation at your home, office, hotel, gym, or another appropriate private location. Musculoskeletal ultrasound and ultrasound-guided treatment may be used when clinically appropriate.

We stay involved.

Follow-up is performed by telehealth when appropriate to assess response, answer questions, and determine what comes next.

Eclosion Health was created by Kareem Qaisi, DO, RMSK, a physician specializing in Physical Medicine and Rehabilitation and Pain Medicine, with advanced expertise in musculoskeletal ultrasound and image-guided procedures. From evaluation through treatment and follow-up, care remains physician-led.

The things people ask first.

Do you take insurance?

No. Eclosion Health is a private, direct-pay medical practice. We do not submit commercial insurance claims. Medicare and other government health programs can involve separate requirements for certain services.

How does pricing work?

Eclosion Health is a direct-pay practice. Exact pricing is provided before care is scheduled based on the treatment, number of treatment areas, and products or materials required. Pricing is designed to include physician review, the house call, the procedure, and routine treatment-related follow-up.

Where does the visit happen?

In-person care can take place at your home, office, hotel, gym, or another appropriate private location. Telehealth may also be used for evaluation or follow-up when appropriate.

Do I need a telehealth visit first?

Not always. We review your questionnaire, history, prior imaging, and goals before determining the best next step. Some patients benefit from telehealth evaluation before the house call, while others may proceed directly to an in-person visit when appropriate.

Can I be treated during the first house call?

Often, yes. If the diagnosis is sufficiently clear, the treatment is appropriate, and there are no reasons to delay, an indicated procedure may be performed during the same visit.

Do I need an MRI first?

Not always. Many musculoskeletal problems can be evaluated through history, physical examination, review of existing imaging, and ultrasound when appropriate. If additional X-ray, MRI, laboratory testing, or another study is needed, we will tell you.

Can Medicare patients receive PRP?

PRP may be available to Medicare beneficiaries in selected circumstances. Medicare generally does not cover PRP for musculoskeletal conditions. Before scheduling, Eclosion confirms whether the requested service can be provided on a self-pay basis and provides any required Medicare notice. If the beneficiary elects not to have the PRP service billed to Medicare, Eclosion does not submit a claim for that service.

Do you prescribe pain medication?

The practice is built around diagnosis and high-quality, image-guided treatments. It does not manage chronic opioid therapy. Medication recommendations may be made when clinically appropriate as part of a broader plan.

Can I use my HSA or FSA?

Yes. HSA and FSA funds can generally be used for eligible medical services provided by Eclosion Health. If your plan requires specific documentation, we can provide an itemized receipt for your records.
